About the Role

Referral Coordinators (RCs) play a pivotal role in managing client-related communication and coordination with outpatient practices, residential treatment centers, hospitals, and specialty providers. Specifically, RCs are tasked with greeting clients who are ineligible for care at Charlie Health, making outbound referrals for clients during the intake process, and supporting the partnerships team in cultivating strong relationships with trusted providers nationwide. RCs are instrumental in bridging gaps in the continuum of care.

RCs are responsible for facilitating outbound referrals based on needs identified during the intake process. These may include referrals to lower or higher levels of care, nutrition professionals, pain management specialists, and external psychiatry and/or medication management providers. Additionally, RCs are accountable for guiding clients and their families through the process of establishing care with providers and facilitating connections with the care team.

To ensure that every client at Charlie Health receives the necessary care, Referral Coordinators collaborate closely with our partnerships and outreach team. Together, these teams forge relationships with local and national resources to establish a robust referral network comprising high-quality providers. RCs participate in regular meetings with our partnerships and outreach teams to pinpoint potential gaps in our referral network and devise strategic solutions.

Responsibilities

  • Ensure a supportive, positive experience for clients and families by coordinating care with referral sources and outside providers
  • Make accurate and timely outbound referrals (PCP, nutrition, psychiatry, pain management, etc.) for non-admitted clients
  • Collaborate closely with internal stakeholders at Charlie Health (e.g., clinical team, admissions team) as needed to fulfill job responsibilities
  • Document all care coordination interactions in the electronic record system
  • Work closely with the Clinical Outreach and Partnerships teams to build a deep understanding of referral sources and the services they provide
  • Adhere to stated policies and procedures and achieve performance metrics goals

Requirements 

  • Bachelor’s degree in health sciences, communications, psychology, social work, or related field
  • Minimum of 2 years of relevant work experience (e.g., experience in healthcare, preferably in customer / patient-facing roles such as case management, discharge planning, referral relations, admissions, or outreach)
  • Strong interpersonal, relationship-building and listening skills
  • Metrics- and results-oriented mindset, with experience working against concrete targets
  • Excellent written and verbal communication skills 
  • Extreme organization and attention to detail
  • Work authorized in the United States and native or bilingual English proficiency
  • Ability to thrive in a fast-paced environment and learn quickly
  • Proficient in Salesforce and Google Suite/MS Office
