Your role as Care Coordinator

Dialogue is seeking a Care Coordinator to join our team on a 3-month, fixed-term contract to help improve the wellbeing of our members and to support the administrative needs of our healthcare team. In this role, you will focus on essential tasks such as managing appointments, processing prescriptions, and providing general administrative support. What you’ll be doing: 

  • Scheduling appointments for patients and healthcare providers
  • Faxing prescriptions to pharmacies and other healthcare facilities
  • Providing administrative support to the care team
  • Assisting with the organization of patient appointments and follow-up reminders and ensuring that all appointment schedules and prescription requests are handled in a timely manner
  • Maintaining accurate records of all scheduling and prescription activities 

We'd love to hear from you if you have:

  • Strong organizational skills and attention to detail
  • Good time management and the ability to prioritize tasks efficiently
  • A customer service-oriented mindset and a willingness to help others
  • Strong communication skills, both written and verbal
  • Strong computer skills
  • Available to work a minimum of 21 hours on evenings and weekends, as our schedule covers both day and evening shifts (6 AM to midnight, weekdays and weekends)
  • Experience in a healthcare or administrative role is an asset

Please note that as we serve customers across Canada, bilingualism is essential for this position. You may be required to communicate in French and English.
