The Family Mentor is an integral member of a patient’s multidisciplinary care team. The role of the Family Mentor is to ensure that caregivers successfully integrate into Equip treatment by adhering to their care plans and fully engaging in treatment. This goal is achieved through proactively identifying and addressing barriers to treatment, identifying opportunities to personalize Equip’s programming to address family needs, and building caregiver skills. The Family Mentor is accountable for balancing patient engagement in treatment and adherence to the care plan with patient satisfaction and retention in treatment. The Family Mentor is motivated by the ultimate goal of providing caregivers with access to the tools and services they need so that their loved one can achieve lasting recovery.
Provide skills-based individual and group sessions to caregivers in treatment at Equip.
Implement motivational strategies with caregivers to engage with their care plans, including monitoring adherence to care plans and completion of program elements.
Participate in multidisciplinary treatment team huddles to support patient progress in treatment.
Actively listen and guide caregivers to anticipate challenges and engage in coping-ahead strategies.
Maintain an availability calendar allowing caregivers to schedule appointments.
Conduct proactive outreach (phone calls/messaging), missed appointment recovery, reminders, tracking, and support to ensure full engagement with the care plan.
Engage in problem-solving to ensure care plan adherence, including identifying and providing tangible solutions to ease day-to-day tasks and stresses.
Ensure personalization of care plans by making program recommendations and coordinating services to ensure caregiver needs are met - examples include, but are not limited to, booking additional care team member sessions and registering for groups; preparing and ordering customized grocery lists; and referring personalized resources to support family.
Guide caregivers in developing advocacy-related documentation, including 504 Plan accommodation request letters.
Build a relationship of respect and trust with the caregiver.
Complete timely documentation of caregiver interactions in compliance with Equip policies.
Attend and participate in internal meetings as well as initial and ongoing training as assigned.
Perform other duties as assigned.
2+ years of experience working with families/supports in a health care setting including, but not limited to, coaching, case management, mentorship, social work, or milieu therapy.
Lived experience as an eating disorder support for a loved one in firm/stable recovery.
Ability to connect with a range of diverse people with varying lived experiences.
Ability to identify and direct patients and families in achievement of treatment goals.
Passion for evidence-based care and ability to learn, practice, and develop new coaching skills.
Strong oral and written communication skills with the ability to build effective, professional, and collaborative relationships both internally and externally.
Strong organizational and time management skills with a proven track record of meeting goals.
Strong attention to detail.
Comfortable working with and learning new technologies
Comfortable working remotely (including the requirement to be on camera).
Comfortable with ambiguity and ability to think critically to quickly and accurately work through challenges in a fast-paced, fluid environment.
Ability to provide 26-30 hours a week of direct patient care, including new patients and follow-up sessions.
Ability to commit to working two evenings until 8pm of your choice per week.

Loan Repayment:
$500/quarter for employees under a year of tenure.

Continuing Education:
Robust training in evidence-based treatments for eating disorders, including Family-Based Treatment, Enhanced Cognitive Behavioral Therapy, and Cognitive Behavioral Therapy for ARFID. Comparable training workshops and supervision in these modalities are valued over $3,000.
For therapists and medical providers, unlimited online courses through NetCE.
For all providers, up to $1,000/year stipend for continuing education.

Work From Home Additional Perks:
$50/month stipend added directly to an employee’s paycheck to cover home internet expenses.

Physical Demands
Work is performed 100% from home with no requirement to travel. This is a stationary position that requires the ability to operate standard office equipment and keyboards as well as to talk or hear by telephone. Sit or stand as needed.
